At Thorpe Chiropractic Office, we are a non-participating provider with insurance plans. That means we do not contract with commercial insurance companies. This allows us to focus entirely on your care without restrictions or unnecessary treatment requirements.

Payment & Insurance Information

Thorpe Chiropractic Office is a non-participating provider with insurance plans, including Medicare.

What this means:

  • Payment is collected at the time of service

  • We can provide documentation (Itemized Bill) for you to submit to your insurance carrier if desired

  • Some plans may reimburse you directly depending on your benefits

We believe in transparency. Fees are discussed clearly before treatment begins so there are no surprises.

Secure Patient Intake Form Instructions

To help make your visit more efficient, we offer secure online forms through our HIPAA-compliant patient portal.

  • Please give our office verbal permission to send your forms by email.

  • Provide the email address where you would like the forms sent.

  • Once the front desk sends the link, you should receive it within approximately 3 minutes.

  • If you do not see the email, please check your spam or junk folder.

  • Open the email, click the link, and complete the requested steps.

  • If you are completing the forms on your phone and the page freezes, refresh the page and continue where you left off.

  • Some questions, such as vaccination information, may not apply to our office and can be skipped.

  • On the last page, titled “Documents,” you will find your PHQ and Back and/or Neck Index forms. Please complete them fully and submit them.

  • If any required information is left incomplete, it will need to be finished in the office before your appointment.

Preparing For Your First Visit

For your first appointment, please download and fill out our Demographic and Insurance document, PHQ (Patient Health Questionnaire), and the appropriate section Index of the spine.

 

Additionally, please bring your insurance card with you. For cash-based patients, please be ready to pay at the time of care.

Your first visit is designed to be thorough but efficient.

It includes:

  • A detailed discussion of your symptoms

  • Review of relevant health history

  • Orthopedic and movement evaluation

  • Clear explanation of findings

  • Discussion of appropriate treatment options

You will understand what is going on and what the plan is before we proceed.
