Payment & Insurance Information
Thorpe Chiropractic Office is a non-participating provider with insurance plans, including Medicare.
What this means:

Payment is collected at the time of service

We can provide documentation (Itemized Bill) for you to submit to your insurance carrier if desired

Some plans may reimburse you directly depending on your benefits
We believe in transparency. Fees are discussed clearly before treatment begins so there are no surprises.

Some questions, such as vaccination information, may not apply to our office and can be skipped.

On the last page, titled “Documents,” you will find your PHQ and Back and/or Neck Index forms. Please complete them fully and submit them.

If any required information is left incomplete, it will need to be finished in the office before your appointment.
Preparing For Your First Visit
For your first appointment, please download and fill out our Demographic and Insurance document, PHQ (Patient Health Questionnaire), and the appropriate section Index of the spine.
Additionally, please bring your insurance card with you. For cash-based patients, please be ready to pay at the time of care.
